Poppy was just 11 when she was diagnosed with a brain tumour. She was in so much pain she could barely lift her head.

But Poppy is a fighter and is determined to not let the brain tumour beat her. Two years on from her diagnosis she is sharing her story in order to help and inspire others.

This Christmas, Poppy is helping us launch our fundraising appeal and hopes that she will be able to raise awareness of the devastating disease.

Gift Aid Declaration

I confirm I am a UK tax payer and I agree to Brain Tumour Research claiming tax on all past present and future donations I make to the charity.

By ticking this box I am confirming I have paid or will pay an amount of Income Tax and / or Capital Gains Tax for each year (6th April to 5th April) that is at least equal to the amount of tax that all charities or Community Amateur Sports Clubs (CASCs) that I donate to will reclaim on my gifts for that tax year.

I understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year it is my responsibility to pay any difference.

I understand that other taxes such as VAT and Council Tax do not qualify.

I understand that Brain Tumour Research will reclaim 25p of tax on every £1 that I give on or after 6th April 2008.
